THE PROCESS:

1 - There will be an INITIAL form (as below) that collects the Young Person's name and the basic details for their Parent or Guardian.

2 - Completion of this form will then trigger an email to the Parent/Guardian with a link to the main form - and the main form  can then be completed by them. A follow up phone call will be made to the parent to start to build the relationship between the club and the family.
